During the service, the sanctuary and the bar/bat mitzvah student’s home (if you are leading from home) will be on screen for the entire length of the service. Others will be brought in from the webinar when it is time for their honors. If you would like for any immediate family members (parents, siblings, etc.) who are not with you to be on the screen for the entire service as well, please list their names/relationship below:
Child's name as you wish for it to appear on the front cover of the program. (First, Middle, Last)
Child's informal first name for the program.
Use transliteration and do the best you can! If you were not named at Schaarai Zedek and you have a certificate, please email a copy of it to Rabbi Weiss at mweiss@zedek.org.
Name of presenter and relationship to child
(Example: Jacob Cohen - Grandfather)
If the child will be given a tallit, the service begins with a presentation. This is also an opportunity for a short (less than one minute) speech to the child. While the parents can do this, parents will still have an opportunity for their main speech later in the service.
Names of child’s parents and grandparents (if desired):
While most students use our yad for their Torah reading, some families give the bar/bat mitzvah their own yad. This can be another opportunity for a very short (under 30 seconds) talk to the bar/bat mitzvah.

Name of Saturday Pulpit Guest
(NOTE: A Board member may be invited by the family; otherwise, the default Board member for your date will attend.
Please let the clergy assistant know ASAP if you have confirmed a Board member of your own preference so we may notify the default pulpit guest he/she will not be required.)
Approved Pulpit Guest List
Names and relationships of those family members who have passed away that you would like to be mentioned prior to Mourners’ Kaddish.
Families are invited and encouraged to join us at the Erev Shabbat service the Friday night before your bar/bat mitzvah service. The student will have a small part in the service and is also invited to join their parents and siblings in leading the candle blessing and/or kiddush. (If there are two services on a given Saturday, the morning family will recite the candle blessing, and the afternoon family will recite the kiddush). Please indicate if you plan to join us and let us know the names of the immediate family members who will participate as you would like them listed.
